It ran in the New England Journal of Medicine in 2025. At 48 weeks, average weight loss was about 11% on 4 mg and about 14% on 6 mg, versus essentially flat on placebo, a statistically significant gap. Cardiometabolic markers improved too, and liver fat dropped substantially, the fingerprint of that glucagon mechanism doing its job [1].

A separate, higher-dose trial called GLORY-2 pushed to 9 mg over 60 weeks and reported average weight loss around 18.6%, climbing to roughly 20% among people who completed the full course [5]. That’s in the same range as the most effective weight-loss drugs currently approved anywhere.

The result that should carry the most weight, though, is a direct comparison. DREAMS-3 put mazdutide 6 mg head-to-head against semaglutide 1 mg in Chinese adults with type 2 diabetes and obesity. On the primary goal, reaching HbA1c under 7.0% and losing at least 10% of body weight, mazdutide hit that mark in 48.0% of participants versus 21.0% on semaglutide, with greater weight loss overall [6][7]. A head-to-head win is a different order of evidence than comparing results across separate trials, and this one is on the record.

What’s the catch? Where should someone be cautious?

A few places. Almost all the pivotal safety and efficacy data so far comes from trials in Chinese populations [1][6]. That’s rigorous evidence for those populations, but drug response in obesity can shift across different groups and diets, which is exactly why regulators elsewhere run their own review before signing off.

The side effects track the rest of this drug class: nausea, vomiting, and diarrhea, mostly during dose increases, and in the head-to-head trial gastrointestinal side effects were somewhat more common on mazdutide than on semaglutide [1][6]. Adding glucagon-receptor activity also raises its own questions, like effects on heart rate and liver enzymes, that a careful regulatory review has to weigh. This isn’t a drug anyone should be managing from an unlabeled vial bought online.

And a good chunk of what’s known still lives in trial reports and conference presentations rather than a fully published long-term record across every dose and use. None of that erases the core finding. Can someone in the US actually get mazdutide right now?
